- Section 1 captures demographics for cross-tabs.
- Section 2 maps cleanly to ISO 25010 sub-characteristics.
- Section 3 collects qualitative themes.
- The survey is one printed page; longer surveys lose responses.
- The codebook (used for analysis) goes in a sub-appendix or as the next page.
